Requirements for Renewal

Head to any BDO branch and submit the following: 
1. Properly accomplished ATM Debit Card A4 form
2. Photocopy of one (1) valid ID 
3. Expiring/expired Debit Card for replacement
 
If physical card is no longer available, submit Affidavit of Loss (notarization not needed) and Questionnaire Form.

Frequently Asked Questions

Replacement fee is FREE if you request as early as six (6) months before the card expiry date, or within thirty (30) days  after the expiry date.  

A P150 replacement fee will be charged per card by the Branch if the request is made BEYOND thirty (30) days after the expiry date. 

You may receive your card on the same day by selecting one of the following options:
 
1. Generic card (with no name on it): Available at any BDO branch
2. Personalized card (a card with your name on it): Available at select BDO branches listed here. 
 

If you prefer to request for a personalized card at a branch not included in the list, please expect a processing time of 5 banking days (for Metro Manila) or 2-3 weeks (for Provincial).

1. Contact your Branch of Account (BOA). Your request for Debit Card replacement shall be processed only by your BOA. You may check contact details of your BOA here. 
 
2. Submit the scanned copies of the following requirements using your bank-registered email address: 
   • Properly accomplished ATM Debit Card A4 Form
     (available in the BDO Website > Debit Cards > Explore > Forms)
   • Photocopy of one (1) valid ID (i.e., Overseas Filipino passport or any valid ID)
   • Expiring / expired Debit card for replacement
      *Submit Affidavit of Loss (notarization/consularization/apostilization not required) and Questionnaire Form if physical card is no longer available 
 
3. An authorized representation may claim your replacement card on your behalf upon submission of a Letter of Authorization. You may use the template here. 

Card activation or change of ATM PIN can only be done at BDO ATMs within the Philippines. You may search for the nearest ATM here.

No. Expired cards are no longer valid for use in stores and online and are required to be surrendered at the branch for proper disposal.

If the expired card is still linked to online shopping and booking apps, please make sure to update with the new card details.
