    • Client Investment Suitability (CIS)
    • Risk Disclosure Statement (RDS)
  2. Trust Agreement
  3. Investment Instruction Letter
  4. Supplemental Agreement (if any)
  5. BIR Tax-exemption Certificate (if applicable)
  6. Signature Cards
  7. Secretary’s Certificate of the Board Resolution on the following: Appointment of BDO as Trustee and Appointment of Authorized Signatories of the Trust Account.
  8. Photocopy of Certificate of Filing, Articles of Incorporation and By-Laws.
  9. Photocopy of two (2) valid IDs for each signatory
