For Partnership - Partnership Resolution
For Corporation - Secretary's Certificate

2. Accomplished enrollment forms and signed CMS Master Agreement*

*Both documents are available at any branch near you. You may also email us at cms-sme@bdo.com.ph to accomplish the documents anytime, anywhere at your convenience.

Submit your application

Submit your completed documents to your BDO Branch of Account, addressed to CMS SME.
