IMPORTANT REMINDER FOR SUBMISSION OF DOCUMENTS: This should not be construed as an admission of liability on the part of the insurer, this is merely a request to gather information that might be their basis in evaluating the claim. The insurer reserves the right to request for additional documents and/or information when the need arises

Claim Requirements

Review the requirements that are needed when filing a claim. 

  1. Original Detailed Incident Report / Police Report (if involved in vehicular accident)
  2. Original Official Receipt/Invoice of Medical Expenses
  3. Original Medical Certificate with Diagnosis
  4. Copy of Medical Prescription
  5. Copy Medical/Laboratory Test Results
  6. Original Certificate of Employment
  7. Copy of Hospital Hospital Bill, Charge Slip, Statement of Account
  8. Copy of valid Driver’s License ID and vehicle’s ORCR, if involved as driver in a vehicular accident

  1. Original Detailed Incident Report / Police Report
  2. Original Birth Certificate
  3. Original Official Receipt/Invoice of Burial Expenses
  4. Original Certificate of Marriage / Certificate of No Marriage
  5. Original Certificate of Employment

  1. Detailed incident report and/or Police report (for vehicular accident) 
  2. Copy of 2 valid IDs and/or Driver's License WITH 3 specimen signatures 
  3. Original Medical Certificate 
  4. Original Official Receipt (Hospital bill, medicine, doctor's fee, etc.) 
  5. Copy of certificate of employment / proof of membership

  1. Detailed incident report and/or Police report (for vehicular accident) 
  2. Copy of school ID 
  3. Original Medical Certificate 
  4. Original Official Receipt (Hospital bill, medicine, doctor's fee, etc.) 
  5. Birth certificate 

  1. Detailed incident report and/or Police report (for vehicular accident) 
  2. Copy of 2 valid IDs and/or Driver's License WITH 3 specimen signatures

  4. Final SOA (for us to see both the admission and discharge dates) 
  5. Copy of certificate of employment / proof of membership

  4. Birth Certificate (Certified true copy) 
  5. Death Certificate (Certified true copy) 
  6. Original Medical Certificate/records (if hospitalized) 
  7. Original Receipts (invoices of funeral/burials expenses/service contracts)  
  8. Marriage contract/birth certificate (of deceased and or children), if applicable (Certified true copy) 
  9. Other evidence to establish relationship between victim and claimant 

  1. Repair Estimate
  2. Pictures showing the damage portion and plate number of the vehicle
  3. Photocopy of Certificate of Registration and Current Official Receipt
  4. Photocopy of Drivers ID License and Official Receipt
  5. Police Report (required for Theft and complicated case) or
  6. Notarized Affidavit (Self accident only with no adverse party)

  1. Repair Estimate
  2. Pictures showing the damage portion and plate number of the vehicle
  3. Photocopy of Certificate of Registration and Current Official Receipt
  4. Photocopy of Drivers ID License and Official Receipt
  5. Certificate of No Claim (from TP's insurance) or Affidavit of No comprehensive insurance (If no insurance)

  1. Repair Estimate
  2. Pictures showing the damage portion / item
  3. Details of Owner with Contact numbers
  4. Proof of Ownership

The CTPL policy will respond first and any excess w/c will not be covered, the Excess Bodily Injury Cover of the Comprehensive Policy will respond.

  1. Medical Certificate
  2. Official Receipt
  3. Signed release of claim and affidavit of desistance
  4. I.D. of claimant

  1. Formal Notice of Claim
  2. Pictures showing the portion/s that were damage or affected by the incident
  3. Copy of Incident/Security/Police Report/Barangay Report and/or Affidavit of the events that lead to the loss
  4. Fire Investigation Report/s issued by the Fire Bureau
  5. DTI Registration of Business name or SEC Certification and business license/permit (if engaged in a business)
  6. Accomplished and signed Sworn Statement of Claim and Non Waiver Agreement

  1. Copy of contractor’s detailed repair estimate showing the scope of work, materials to be used with qunatity and unit cost and labor cost
  2. Copy of reconstruction cost (case to case)
  3. Copy of repair/replacement invoices (if already repaired)
  4. Copy of Transfer Certificate Title or Declaration of Real Property Tax / Copy of Lease Contract (if lot is not owned)

  1. Detailed inventory of affected and unaffected items with at-cost price
  2. Copy of Income Tax Return with audited financial statement for the last three(3) years prior the loss
  3. Monthly stock position report for the last 6 months prior the loss showing the beginning balance, stocks received, stocks sold and ending balance.
  4. Copy of Books of account, purchases and sales invoices
  5. Documents to support unit costings used
    • System audit that will justify actual inventory at the time of loss and the at cost price. If above are not available  

  1. Detailed inventory of affected and unaffected items with purchase price and date of purchase
  2. Copy of Quotation from supplier
  3. Copy of repair/replacement invoice (if already repaired)
  4. Copy of Technical report on cause of loss, extent of damage and recommendation if repair or replacement

  1. Original copy of your claim letter against Insurer stating amount of loss
  2. Original received copy of your claim letter against Carrier; Arrastre; Broker/Forwarder;
  3. Original copy of Bill of Lading
  4. Original copy of Commercial Invoice
  5. Original copy of Packing List
  6. Original copy of Broker’s Delivery Receipt
  7. Copy of Equipment Interchange Receipt
  8. Detailed Incident Report / Marine Protest
  9. Colored photos of the damaged cargo
  10. Bad order certificate (if any)
  11. Your formal position on the affected cargo whether you intend to retain or surrender to insurer

  1. Original copy of your claim letter against Insurer stating amount of loss
  2. Original received copy of your claim letter against Carrier; Arrastre; Broker/Forwarder;
  3. Original copy of Bill of Lading
  4. Original copy of Commercial Invoice
  5. Original copy of Packing List
  6. Certificate of Cargo Discharge from the vessel
  7. Tally Sheet discharged from the vessel
  8. Tally Sheet received by your company
  9. Statement of Facts
  10. Copy of Load Port/Discharged Port Survey
  11. Delivery Receipts / Waybill
  12. Photos showing the affected cargo
  13. Detailed Incident Report / Marine Protest
  14. Your formal position on the affected cargo whether you intend to retain or surrender to insurer

  1. Detailed Incident Report
  2. Detailed Inventory of damaged items with documents to support the costing applied
  3. Detailed Repair Estimate / Replacement Quotation
  4. Proof of Ownership
  5. Colored Photos of damaged items/portion

  4. Copy of ORCR of involved vehicle
