Motor Claim Requirements

- Repair Estimate
- Pictures showing the damage portion and plate number of the vehicle
- Photocopy of Certificate of Registration and Current Official Receipt
- Photocopy of Drivers ID License and Official Receipt
- Police Report (required for Theft and complicated case) or
- Notarized Affidavit (Self accident only with no adverse party)
- Repair Estimate
- Pictures showing the damage portion and plate number of the vehicle
- Photocopy of Certificate of Registration and Current Official Receipt
- Photocopy of Drivers ID License and Official Receipt
- Certificate of No Claim (from TP's insurance) or Affidavit of No comprehensive insurance (If no insurance)
- Repair Estimate
- Pictures showing the damage portion / item
- Details of Owner with Contact numbers
- Proof of Ownership
The CTPL policy will respond first and any excess w/c will not be covered, the Excess Bodily Injury Cover of the Comprehensive Policy will respond.
- Medical Certificate
- Official Receipt
- Signed release of claim and affidavit of desistance
- I.D. of claimant
