Dispute Form
  • This form is for existing BDO account or cardholders who want to file new disputes regarding BDO transactions on their account/ card. Please prepare documentary requirements for uploading in this form to support your dispute claim.
  • For dispute follow-ups, call our contact center at (02) 8888-0000.
  • No need to fill out this dispute form for the following scenarios:
    • Lost or stolen cards or online banking transaction issues: Instead, call our contact center immediately at (02) 8888-0000 for account blocking and card replacement.
    • Unsuccessful/ incorrect Instapay transactions: Instead, monitor both source and destination accounts for reversal or posting within three (3) banking days from transaction date.
  • If you did not make the transaction being disputed (i.e. transaction is not known to you), BDO will automatically block your card/s within 3-5 days after receipt of submitted form.
    • Call our contact center at (02) 8888-0000 for credit card replacement; for debit card replacement, please visit your branch of account.
