No need to fill out this dispute form for the following scenarios:
Lost or stolen cards or online banking transaction issues: Instead, call our contact center immediatelyat (02) 8888-0000 for account blocking and card replacement.
Unsuccessful/ incorrect Instapay transactions: Instead, monitor both source and destination accounts for reversal or posting within three (3) banking days from transaction date.
If you did not make the transaction being disputed (i.e. transaction is not known to you), BDO will automatically block your card/s within 3-5 days after receipt of submitted form.
